Lambeth Adult Social Care Offer for adults with a disability

Adult Social Care supports Young People when they reach their 18th birthday.

Lambeth Adult Social Care is committed to the highest quality of service for our residents who need care and support. Our aim is to work with the people of Lambeth and our partners to develop community services that enable all adults to live as independently as possible. Supporting them to make the changes to their lives that they want to, with the least amount of intervention and interference.

This means that we go beyond 'wanting to do the right thing' to genuinely engaging with and listening to citizens. We work to understand their needs and help them to make decisions about the services they want and need. We will enable them to develop solutions for themselves. Wherever possible we will collaborate with citizens to co-design and co-deliver services in the Borough.

For Adult Social Care this means that we will promote health and well-being in the following ways: Invest in services that are universal and ensure that citizens have good information on how to access them easily and to manage with the least amount of interference.

Focus Adult Social Care interventions to support people to recover, recuperate and rehabilitate.

Ensure an absolute focus on safety to support the most vulnerable citizens in our community.

We have an Integrated Disabilities Service supporting people with disabilities.This Service supports people who: Have learning and/or physical disabilities that emerge in childhood and that require statutory support from the children and family division, and who go on to require some form of intervention as adults under national guidance and regulations. Have lifelong conditions whose needs may emerge or increase during their adult life. This will include people with conditions such as Autism or Huntingdon's disease.

Acquire a disability between the ages of 18 and 65, including those with acquired brain injuries, HIV/AIDS or sensory impairment.

Support is also available to adults with mental health or substance misuse issues. The Mental Health Service, integrated with the South London and Maudsley NHS Foundation Trust., supports adults experiencing mental ill health. The Substance Misuse Team supports adults who misuse drugs and/or alcohol.

We will assess whether a young person is likely to have needs after turning 18, and what these are. We will also assess a young person's carer's needs if it appears these needs will continue after the young person turns 18.A range of leaflets setting out the eligibility criteria can be found on the Council website http://www.lambeth.gov.uk/social-support-and-health/social-care-and-support/community-care-and-needs-assessment-guide

We aim to have adult assessments completed before a young person's 18th birthday secure the social care support from age 18 and contribute to EHC planning where that is needed.

Where adult assessment has not been completed when a young person has turned 18 their current children's social care provision would continue post 18 until a new assessment has been completed. Costs depend on which services we arrange:
